When schools begin to reopen for eligible pupils from 1st June, do you intent to:
Continue to keep your child at home and make use of the home learning materials provided by school
Return your child to school, following the school's detailed guidelines
If your child has been attending school as a Key Worker or vulnerable child as is in an eligible year group to return on June 1st (Year N, R 1 and 6), would you prefer your child to
remain in their Key Worker group - therefore retaining continuous access to the service as well as before and after school provision as required
transfer to a 'bubble' group in their current Year group, where they will attend 1 week out of each 3 weeks, on a rota basis (they are not then able to return to the Key Worker group)
If your child will be returning to school AND you are already registered with either the breakfast or after school provision would you (there is no guarantee that this may be available)
be requesting the use of breakfast club provision
be requesting the use of after school club provision
not require either breakfast or after school provision
Please tick each of the following to confirm that you have read and understood the school's planned arrangements for re-opening.

I am aware that when the school reopens, should I wish my child to return, my child will be allocated into a group of up to 10 pupils who will attend school on a 3 weekly rota basis (i.e. in one week, at home for two weeks)
I am aware that a school adult will be assigned to a group of children and it may not be their usual teacher or TA who is responsible for them
I am aware that there will be strict arrangements for the dropping off and collection of pupils including specific times for each year group to arrive and be collected - and that children who arrive at times outside of these will not be allowed into school that day
I am aware that strict social distancing and hygiene arrangements will be in place around school which will mean that the school day is significantly different to what children remember
I am aware that once my child has been allocated to a 'bubble' group, they will not be able to mix with children from other groups (including key worker groups)
I am aware that before and after school provision may only be available for children in the key worker groups and not those within their year group 'bubbles'
I am aware that the curriculum delivered in school will not be the normal curriculum and we are unable to provide transition activities for pupils in Years N-5
I am aware that parents and carers will not be allowed on to the school site and that the only way to contact the admin office will be via email or telephone
